Self Driving car insurance

I've thought about this a lot. Who pays for the health costs of an individual when a failure happens on the roads when cars all drive themselves? The obvious answers seem to fail upon examination: the software creator, the automobile creator, the passenger of either vehicle, etc. There would need to be some sort of universal insurance pool that everyone pays into.

If it's a Google self driving car, Google needs to cover the insurance because the vehicle is considered the driver. If they're that safe, perhaps a portion of the purchase cost would cover long term insurance? Hadn't thought about car owners paying Google an annual fee for insurance, but s'pose anything's possible...
