Why did May remove the ladder? I personally think she wanted Toru to see how dependent he was on her.

And this is kinda' metaphorical, 'cause he was becoming dependent on her - she was his social life, the only person that seemed to have any real interest in him.

The thing with the women - Malta and Creta - that was mostly fantasy, I think.
